She was predeceased by: her parents, Joseph Kulisek and Mildred Kulisek; her husband Roy J. Forsythe; her daughter-in-law Amy Forsythe; and her grandson Roy Jason Forsythe.
She is survived by: her children, Roy Forsythe (Barbara), John Forsythe (Wendy), Paul Forsythe (Catherine), Betsy Winters (John), Kristin Poremski (John), Peter Forsythe (Edith), David Forsythe (Alexis), Kevin Forsythe (Carmina), Dan Forsythe (Heather) and Katie Asch (Justin); and her siblings, Laverne Jaros (Mark) and Joe Kulisek (Virginia). She is also survived by many nephews, nieces and cousins.
Visitation will be held on Wednesday, May 7th 2025 from 9:30 AM to 10:30 AM at the St. Thomas the Apostle Catholic Church (1500 Brookdale Rd, Naperville, IL 60563). A Mass of Christian burial will be held on Wednesday, May 7th 2025 at 10:30 AM at the same location. A committal service will be held on Wednesday, May 7th 2025 at 1:00 PM at the Abraham Lincoln National Cemetery (20953 W Hoff Rd, Elwood, IL 60421).
