He is survived by: his parents, John Middleton Sr. (Barb) and Susan Van Kirk (Rodney Aney); his children, Brett Michael Middleton, Logan Makenzie Johnson and Charleigh Kathleen Johnson; his wife Crystal Middleton; his grandmother Beverly Heilman; his granddaughter Sage Middleton; his siblings, Angela Loeffelholz (Todd), Jason Aney, John Middleton Jr. (Mars) and Troy Middleton; and his pet Farrah. He is also survived by many nieces and nephews and man's best friend.
Visitation will be held on Saturday, September 6th 2025 from 10:00 AM to 12:00 PM at the Lanham-Schanhofer Funeral Home and Cremation (106 W Franklin St, Sparta, WI 54656). A memorial service will be held on Saturday, September 6th 2025 at 12:00 PM at the same location. A celebration of life will be held on Saturday, September 6th 2025 at 12:30 PM at the Wisco (2700 Riley Rd, Sparta, WI 54656).
