He was predeceased by: his parents, Joseph Church and Martha Church (Summers); his stepfather Cecil Harsh; his wife Paulette J. Church (Prunty); and his sisters, Barbara Sauro, Mary Buchannon, Clara Layman and Betty Watkins.
He is survived by: his children, Paula Frase (Jeff) and Jeff Church (Lynette); his grandsons, Joshua Frase (Abbey), Christopher Frase, Tyler Church and Anthony Church; and his stepbrothers, David Harsh, Gary Church and Anthony Church.
